Outpatients
Various Outpatient Clinics are held at the Cedarwood Centre where patients attend for an initial appointment and any follow-up appointments if needed as a 1-1. This is either facilitated via members of the Outreach Team or within our dedicated medical outpatient clinics.

- Advance care planning
- Spiritual and pastoral
- Social and welfare
- Family support
- Celebration of life/legacy work (memory boxes/letter writing etc)
- Complementary Therapy
Medical Outpatient Services
Our Medical Team holds clinics on various days during the week. Patients are referred to our Medical Outpatients department by their Consultants, GP, or Community Nurse Specialist when they feel we can make a difference and improve the care a patient receives.
Patients are seen at initial or follow up appointments by our Consultants in Palliative Medicine or one of our team of experienced doctors who review symptoms, monitor their condition, and discuss and plan their care. We actively encourage family and carers to be involved in our patient’s care so they can help to support their loved ones at home.
After each appointment, our Medical Team liaises with other professionals who are closely involved in the patient’s care e.g. GP’s, other Consultants, Community Nurse Specialists.
